Key Responsibilities
Care Management
- Develop and review individual care plans
- Conduct client assessments
- Match carers to clients based on needs and compatibility
- Monitor care quality and client satisfaction
- Handle complaints and concerns professionally
Team Coordination
- Schedule and roster care staff
- Support carers with complex cases
- Conduct supervision and performance reviews
- Identify training needs
Stakeholder Relations
- Liaise with families, GPs, and healthcare teams
- Build relationships with referral sources
- Represent 360 Living professionally
Requirements
Essential
- Healthcare or social care degree (or equivalent experience)
- Proven coordination/management experience
- Understanding of Irish healthcare landscape
- Strong IT proficiency
- Excellent written and verbal communication
Desirable
- Knowledge of HSE funding processes
- Experience with care management software
- Background in domiciliary care
- Management qualification
